not issue uttradhikar certificate by tahsil office

My father Late Mr. Shamsuddin has expired on 6 march 2011. He was retired from LIC gorakhpur. My mother (Mrs Shahnaz Begum, R/O Ismailpur Gorakhpur) is auther person to get family pension, due to this LIC pension department asked for Uttradhikar pramar patr which is issued by Tahsil office. We have submitted application and all neccessary document before 4.5 months. All enquiries has been complited. No my application is in tahsil office. It has been around 1 month passed but they are issuing this.
