Sub :- Settlement of my PF Account – GR/CDP/0034791/000/0000208

I have already submitted my PF application 5 months back by Courier through my previous Employer M/s. Sagar Sugars & Allied Products Limited, Nelavoy village, S.R. Puram Mandal, Chittoor District 517167 AP. But I am disappointed to note that still my PF A/c is yet to be settled. For your easy reference once again I am enclosing my fresh PF Application:-

I am enclosing my PF application along with the following enclosures :-

PF Application duly filled and signed by me and my earlier Employer with a covering letter to
Asst. PF Commissioner – (Annexure – 1)

Two sets of Canara Bank SB passbook cover page duly attested. – (Annexure – 2)

Two sets of Ration card copies duly attested – (Annexure – 3)

My earlier PF application is also enclosed for your kind reference – (Annexure – 4)

Also please note that as per SMS message BZ EPO. I have in my above account is EE amt is Rs. 34116/- and ER amount is Rs. 10,428 as on 16.3.2012. Account updated up to 31.3.2011. Balance from 1.4.2011 to 21.9.2012 is also to be settled.after necessary form filed by my earlier employer (i.e.) SSAPL.

I request you to kindly arrange to clear my full and final settlement of my PF account by (from the beginning of my employment in Sagar Sugars on 12.3.2005 to till 21.9.2012) crediting to my Canara Bank Savings Bank Account No. 0911.101.86340 (complete details of Bank and Branch address mentioned in my PF application) Account at the earliest and oblige.
